On Tuesday November 30th, we celebrated Giving Tuesday; a day when the nation recognized and supported nonprofits doing essential work. This year, Giving Tuesday was bigger than ever, but so too is the need!  Your help is still needed!

We Believe…

Everyone deserves a system of care that prevents avoidable illness and suffering.

We can dramatically improve preventive care systems and that doing so is a societal imperative, especially for our most vulnerable populations.

With your donation we’ll be able to:

• Deliver in-home whole-person preventive care services to vulnerable, socially isolated, older adults.
• Sustain our dedicated team of nurses who provide programs to prevent chronic disease and to mitigate health risks.
• Work collaboratively with primary care physicians to adapt and integrate HQP’s model of Advanced Preventive Care (APC) into primary care.
• Improve HQP’s SPERO platform to help care teams take better care of patients.
• Spread effective care models to improve health, longevity, and quality of life for all who can benefit
