- Better manage and prevent the onset or complications from common chronic diseases - heart failure, coronary artery disease, chronic lung disease, depression, and diabetes
- Avoid complications and prevent hospitalizations associated with chronic conditions and aging
- Promote safety and independence and optimize physical and social functioning
- Assist with and facilitate advanced healthcare care planning

Community Based Nurse Care Managers
HQP is seeking talented nurses to provide advanced preventive care. This is a clinical position that provides in-person community based care. The role requires a passion for learning, and a commitment to deliver a better care experience for patients and their families. Role effectiveness requires accountability for developing long term person-centered relationships, completing comprehensive assessments and ongoing monitoring, anticipating problems and collaboratively solving them, seeing patients and coordinating their care across all settings, addressing complex psychosocial problems, and patient education. The ability to effectively collaborate with physicians and other health care providers and community resources is essential. HQP provides extensive initial orientation and training for the role and significant ongoing mentorship for support and ongoing professional development.
REQUIREMENTS:
Education: Minimum: Bachelor of Science in Nursing and RN license required.
Experience: Five years of current clinical experience in the following areas of practice: medical-surgical, home care, hospice, community health, rehabilitation, long term care, cardiovascular, diabetes, and or pulmonary diseases, oncology and geriatrics.
Additional Requirements:
RN license – Pennsylvania and NJ (or NJ eligible)
Must have own vehicle for daily travel in HQP service area with a valid driver’s license and automobile insurance coverage, as required by law
BLS Certification
