National Academy of Medicine Webinar 2018 - HQP's system of Advanced Preventive Care was highlighted in a webinar session on Effective Care for High-Need Patients presented by the National Academy of Medicine and supported by the Peterson Center on Healthcare on March 29. The webinar highlights the insights HQP has gained over the years and some of the practical challenges associated with scaling an effective model of care for chronically ill, older adults. Dr. Ken Coburn of HQP was joined by Dr. Jonathan Harvey, Chief Medical Officer at Martin's Point Health Care who described their organization's early experience adopting Advanced Preventive Care using a system of replication supported by a collaborative partnership with HQP. #HighNeeds

Population Health Colloquium 2014 - HQP's CEO, Ken Coburn, MD, MPH gave the Closing Keynote presentation at the 2014 Population Health Colloquium in Philadelphia on March 19. In his remarks, Dr. Coburn emphasizes the importance of disciplined design and reliable implementation in creating Advanced Preventive Care models for vulnerable populations. 8th Annual HealthCare IT Summit (PA) 2012- On the Importance of Design - At the 8th Annual HealthCare IT Summit in May 23, 2012, Dr. Ken Coburn, CEO and Medical Director of HQP had the distinct privilege to present his thoughts about the role of design in transforming health care. The meeting was jointly put on by the Pennsylvania eHealth Initiative and the Central Pennsylvania Chapter of HIMSS and was held in Harrisburg, PA. Care Innovations Summit 2012 - Highlights of the HQP model of Community-based Care Management were presented by Ken Coburn, MD, MPH at the Care Innovations Summit in Washington., D.C., on Jan. 26, 2012 which was co-sponsored by the Center for Medicare and Medicaid Innovation and the West Wireless Health Institute. Coburn described HQP’s model, and the positive health outcomes and significant cost savings seen among higher-risk populations in the Medicare Coordinated Care Demonstration (MCCD) and in an Aetna (Medicare Advantage) regional collaboration.
