From designing a single physician practice to a state-wide plan for vulnerable populations, HQP has long been at the forefront of creating change through design thinking for systems of care.  System-savvy design is essential for innovating health care delivery across all stakeholders and regions.

Design is the single-most important determinant of the overall quality and effectiveness of population health programs.  

Since 2001, HQP has proven experience designing programs using principles and operational domains that guide the development of new programs in a disciplined manner.  A rare blend of public health, systems, and design thinking combined with an understanding of the challenges and practicalities of delivering health care and social services, and the means to harness large and divergent data sources is required for large system population health transformation.  By dint of its long history of working with these dimensions and undertaking the applied R&D needed to hone Advanced Preventive Care, HQP can be a valuable resource to any organization committed to developing or improving a comprehensive population health management system. 

Projects

Advanced Preventive Care

​Design, implement, and rigorously evaluate Advanced Preventive Care for chronically ill older adults. Since 2001, HQP has created and honed a highly effective and innovative system of nurse-led community-based care management called Advanced Preventive Care (APC) for rising risk patients on Medicare. In a Centers for Medicare and Medicaid Services (CMS) 12+ year randomized controlled trial (RCT), APC was shown to reduce hospitalizations (-39%), deliver net savings (-28%; -$397 PMPM) and lower mortality rates (by -34% at two years, and -22% at five years) among higher-risk chronically ill older adults compared to those receiving “usual care.”1,2 A recent Commonwealth Fund Issue Brief reporting on 18 models that have had the most success in managing complex, high-need populations; identified HQP’s Advanced Preventive Care model as having the highest level of evidence among an elite set of programs.3 APC combines clinical expertise, hands-on care management support, and a technology infrastructure to support provider organizations in replicating APC to improve the overall value of care provided to their most challenging patient populations.
Cancer Center "Hub" Model

​Design and implement a "hub model" of care led by nurse coordinators at the Froedtert Health Clinical Cancer Center to ensure seamless, longitudinal support of cancer patients by a multidisciplinary team. Froedtert Hospital was awarded the 2016 Outstanding Achievement Award by the Commission on Cancer (CoC) of the American College of Surgeons (ACS).
Bundled Payment Care Improvement (BPCI & BPCI-A) Programs

Design and implement 90-day bundled payment programs for patients with heart failure at two different health systems in southeast Pennsylvania; CMS-sponsored programs, Bundled Payment for Care Improvement (BPCI), and Bundled Payment for Care Improvement – Advanced (BPCI-A).
Spread Systems

Designed a set of services and technologies (SPERO(R)) to facilitate the spread and replication of APC through the development and use of Replication Consultancy – a replication system designed by HQP that supports other organizations to adopt and implement APC with reproducible effectiveness. Current replication sites include Martin’s Point Health Care (Portland, ME) - a hybrid provider system and health plan, and Salveo, a care management subsidiary of St. Lukes Health (Tasmania, AU) - a secondary health insurer in Australia.
