Our Mission
In the United States and across the world, non-communicable chronic illness is now the major determinant of the length and quality of human life and is a key driver of health service utilization and cost. Preventing the onset and complications of chronic disease is essential for improving population health. There is good scientific evidence for interventions that help prevent the onset or worsening of many chronic illnesses, but we lack systems that reliably deliver a broad range of preventive interventions to those who most need and can benefit from them.
Better systems of preventive care are key to improving health and controlling health care cost over the long term – especially for vulnerable populations. Such systems are the foundation for population health management. Developing highly effective systems of preventive care requires creativity, disciplined design, research and development, teamwork and collaborative partnering, new uses of data and analytics, and relentless constancy to purpose.

Our Experience
HQP has extensive experience doing applied research and development, and creating and implementing more effective systems of preventive care and population health management. For many years, HQP has directly delivered Advanced Preventive Care to chronically ill older adults across a sizable region of southeastern Pennsylvania. We've done this in collaboration with primary care physicians, specialists, acute, post-acute, and community resources with funding through a variety of sources including CMS, health plans, health systems, private donors, corporate sponsorships, and grants.
