I'd like to thank our collaborative partners that worked with and supported HQP in designing and testing more effective models of health care in 2015. Last year was a pivotal "transition" year for HQP in many respects, being our first year without CMS demonstration funding support in over 12 years. Thankfully, we are still in dialogue with our CMS colleagues, and are exploring other means to further this important research together.
Among our major milestones for 2015; beginning to work with an ACO to help them replicate our model of advanced preventive care as a mainstay of their population health management system, launching our SPERO® application, and continued effectiveness in improving the health of higher-risk Aetna Medicare Advantage members (a collaboration now in its 6th year). There are a host of exciting new opportunities we will be pursuing in 2016, including; some very innovative community health system design work and some major organizational collaborations that are on the drawing board. More to come soon about what we've got teed up for 2016.
